Hours of Operation
Emergency Room: Open 24 hours a day, 7 days a week — always.
Outpatient Clinic: Monday through Friday, 8:00 AM – 5:00 PM.
Insurance & Payment
We accept most major insurance plans. Please call ahead to verify your coverage. Financial assistance may be available for qualifying patients — ask our billing team for details.
What to Bring
A valid photo ID, your insurance card, a list of current medications, and any referral forms your provider may have given you. Arrive 15 minutes early to complete check-in.
Location & Parking
Located at 50 E 200 S in Duchesne, UT 84021. Free parking is available in our lot adjacent to the main entrance. Wheelchair-accessible entrances are clearly marked.
